Floor plan to create an opening with a ladder for multi-story buildings.

Right click the floor of the house, select Plan > Build floor with opening above to create a floor plan
Right click the floor plan, select Build > Slate slab opening

Premium paving skill is required to start a stone slab opening. 21 is assumed, possibly higher.
